Welcome back EVHS & CFVMS Parents/Guardians and Students,
All of the following forms must be completed and submitted through Rank One before any participation in Española Valley High School/Carlos F. Vigil Middle School athletics. No paper documentation will be accepted. (No exceptions.)

1. Athletics Participant Form
• Review the following:
    * Concussion Information Acknowledgment
    * Consent to Treat
    * Parent Code of Conduct
    * Student Code of Conduct
• After reviewing, check the boxes, then sign and submit.

2. Medical History / Physical Upload Form
• Use the NMAA-approved physical form (available under the Download and Print tab).
• Take the form to your child’s physical appointment.
• Upload the completed form under Medical History/Physical Upload Form as either a PDF or a photo.
• Please click on the examples to know which forms to upload out of the packet.

3. Search and start the following required courses:

For the Alcohol/Drug Abuse for Students Certificate
4. Visit: https://lifeofanathlete.talentlms.com/
a. Sign in or create an account.
b. Click on Course Catalog
c. Add the course “Life of an Athlete”
d. Complete the course and after it will ask you to download the certificate.

5. After completing each course, save the certificate (must be in the student’s name and current).
- Certificates must be renewed yearly. Old/expired certificates will be rejected.
- Upload as a PDF or picture.


4. Code of Ethics Acknowledgement Form
• Review the following:
    * Handbook Acknowledgement Form
    * Athlete Code of Ethics
    * Athletic Substance Abuse, Alcohol, and Tobacco Policy
    * Parent/Guardian Code of Expectations for Athletics
• After reviewing, check the boxes, then sign and submit.

5. Emergency Contact Card
• Complete and submit online
